Derm Stumped by ACNE IMPOSTER 🧐 This client visited a dermatologist for a sudden onset of bumps on her chest. She never had acne on her body before maybe the occasional little bump on her face during her menstrual cycle (side note: I always refer out to derms to diagnosis because it’s not in my scope 🤍) In her case they said acne bacteria was present on the skin and recommended a topical liquid solution. She didn’t have improvement and she sought me out. A few clues that helped me identify fungal follicultius vs bacterial acne: ▫️ Uniform sized bumps ▫️ Often itchy ▫️ Clusters of bumps on her chest ▫️ Superficial red papules and tiny whiteheads Fungal Folliculitis aka Malassezia folliculitis is caused by a yeast overgrowth, not Cutibacterium acnes (the bacteria most often linked to traditional acne). 🦠 here’s the nuance: Both Cutibacterium acnes (the bacteria linked to traditional acne) and Malassezia (the yeast behind fungal folliculitis) live on everyone’s skin. Healthy skin. Clear skin. Breakout-prone skin. They’re part of your normal microbiome & they’re supposed to be there. 🔬 when the balance shifts and one of these organisms multiplies inside the follicle it triggers inflammation. It’s easy for the yeast to slip by in traditional testing especially if it’s already assumed the cause is acne.
